• Valid certification issued by the National Commission on Certification of Physician Assistants (NCCPA);
• Certification from the jurisdiction of original licensure and current licensure stating the date of issuance and license status, and any disciplinary actions taken or pending; and
• A $50 application fee.

When the applicant meets the requirements for his or her application, then the physician assistant license will be issued. However, even though the physician assistant may be licensed, he or she may not be allowed to practice until an employment notice has been filed.
